How do I sign up?

Patients who wish to participate will be issued a MyChart activation code during their office visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during your next office visit. You may also visit the Care New England MyChart Login page to sign up as a New User: login page

Who do I contact if I have further questions?

Please contact your doctor's office for further questions.

When can I see my test results in MyChart?

Most testing results will be released to you promptly once they are finalized, therefore you may receive notification of your results before a provider has had a chance to review them.

Why are certain test results not shared electronically via MyChart?

Due to Rhode Island State Law, HIV test results will not be shared with you via MyChart until your provider has had a chance to review the results with you. Providers also have the right to withhold a result from you, on an individual basis, if the Provider determined the release of the information is reasonably likely to endanger your life or physical safety or cause you or someone else substantial harm. You have the right to ask your Provider to withhold a note or a result from your MyChart record.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your provider's office. Ask your provider’s office how to submit a request to amend or correct your medical information. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 3 business days. Please note that MyChart should not be used for urgent situations. Please contact your provider if the situation requires immediate attention or dial 911 if it is an emergency.

Can I view a family member's health record in MyChart?

Yes, you can in certain circumstances. This is called Proxy access and allows a parent (or guardian) to log into their personal MyChart account, and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to your provider to request access to this convenient service.

Can I ask questions regarding a family member from my MyChart account?

No. MyChart offers direct access to your personal health record and thus communicating about a family member’s information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

I forgot my username or password. What should I do?

Please contact your doctor's office to get a new activation code.

I didn't receive my two-step verification code. What should I do?

If the code was emailed to you, try checking the spam or junk folders in your email. If it is not there, try clicking Resend Code. If you have multiple email accounts, check one of the others to see if the verification code was sent there instead. If you still didn’t receive the email or text message, it could be that your account has a different email address or mobile number on file. In this case, please contact your physician’s office to confirm your contact information is up to date.

What do I do if I get locked out of my account?

To have your account re-activated, contact your physician’s office.
